Value of 99Tcm-NOET gated myocardial perfusion on patients with coronary heart disease

Abstract:
Objective To predict the value of 99Tcm-bis(N-ethoxy-N-ethyl-dithiocarbamato)nitridotechnetium(99Tcm-NOET)gated myocardial perfusion SPECT on detecting coronary artery disease.Methods For evaluation of suspected coronary artery disease,45 men and 11 women(mean age,45.9±12.4)underwent the rest gated SPECT.SPECT imasing was began 1 hour after intravenous injection of 99Tcm-NOET at rest.The SPECT images acquired end diastolic volume(EDV),end systolic volume(ESV),left ventricular ejection fraction(LVEF).And were analyzed from end systolicregional perfusion.ejection fraction,motion and thickening by 4 polar maps.All of them underwent coronary angiography(≥50%luminal diameter stenosis was considered significant coronary stenosis).Results Sensitivity and specificity of 99Tcm-NOET rest gated myocardial perfusion SPECT for detecting coronary artery disease were 68.42% and 83.33%,respectively.There wag statistically significant difference for function parameters(EDV,ESV and LVEF)between myocardial infarction group[(129.32±9.14)ml,(80.97±9.49)ml,(40.15±3.28)%]and control group[(80.91±3.12)ml,(30.12±1.79)ml,(63.51±1.04)%,EDV:F=22.103,ESV:F=32.277,LVEF:F=42.604,P<0.01].There were not significant difference between control group and ischemic group[(70.83±3.46)ml,(25.13±2.85)ml,(65.55±2.62)%].In myocardial infarction group left ventricular were divided into 460 segments,and 209 segments were abnormal simultaneously in regional perfusion,ejection fraction,motion and thickening by 4 polar maps.Conclusion s 99Tcm-NOET rest SPECT is clinically feasible in detecting coronary artery disease.Global left ventricular function parameters have higher values in assessment of myocardial infarction and not have advantage in diagnose of coronary artery disease.In the patients with myocardial infarction,not matching between the regional perfusion and wall contraction function is useful to assessment of myocardial viability.
Diagnosis of thyroid carcinoma using 99TcmO4-/99Tcm-HL91 imaging

Abstract:
Objective To evaluate the clinical value of 99Tcm-sodium pertechnetate(99TcmO4-)and 99Tcm-4,9-diaza-3,3,10,10-tetramethylododecan-2,11-dione dioxime(99Tcm-HL91)imaging in differentiation of thyroid nodules.Methods 99TcmO4- dynamic and static imagings were performed to the 65 cases of patients with thyroid nodules.After 48 hours,the 46 cases of patients whose 99TcmO4- static imaging prompt cold nodules underwent 99Tcm-HL91 hypexia imaging.And finally,compare the results of the two types imaging with biopsy results.Results The sensitivity,specificity and accuracy of 99TcmO4- imaging in the diagnosis of thyroid nodules were 78.6%,71.9% and 73.9% respectively.The sensitivity,specificity and accuracy of 99Tcm-HL91 imaging in the diagnosis of thyroid nodules were 88.7%,92.3% and 91.4%respectively.And the sensitivity,specificity and accuracy of Combining 99TcmO4/99Tcm-HL91 imaging were92.9%,93.8% and 93.5% respectively.Conclusion Combining 99TcmO4- and 99Tcm-HL91 imaging methods can improve the coincidence.
The clinic application of Meyer method of pulmonary perfusion imaging in evaluation of the therapeutic effect of pulmonary embolism

Abstract:
Objective To assess the clinic value of Meyer method of pulmonary perfusion imaging in evaluation of the therapeutic effect of pulmonary embolism.Methods Thirty patients who were diagnosed as pulmonary embolism and received anticoagulant or thrombolytic therapy.All patients received pulmonary ventilation/perfusion imaging before treatment within 1 to 2 days and received pulmonary perfusion imaging again after treatment within 1 to 2 weeks.The two images obtained in each patient were scored together.Meyer method of semi-quantitative visual evaluations were used.Results Thirty patients after therapy,among 15 patients obviously improve of pulmonary peffusion,8 patients lightly improve,5 patients no change and 2 patients aggravation.By Meyer method,the 23 patients of improvement group,the score of before treatment(0.45±0.14),the score of after treatment(0.22±0.11),the score was significant ehange(t=11.627,P<0.05).the 7 patients of no change group,the score of before treatment(0.23±0.15),the score of after treatment(0.23±0.17),the score was not signifieant change(t=0.143,P>0.05).the improvement rate of two groups were significant change(t=2.410,P<0.05).Conclusion The Meyer method could effectively evaluate the pulmonary perfusion change after the treatment of pulmonary embolism,and there was clinic value in evaluation of the therapeutic effect of pulmonary embolism.
131I-orthoiodohippurate renogram exploring the relationship among the radionuclide renography,hydronephrosis and renal function

Abstract:
Objective To investigate the 131I-orthoiodohippurate renography with hydronephrosis and renal function,in order to further clarify the clinical diagnosis and treatment of hydronephrosis and provide a reliable basis.Methods Retrospective analysis the results of 131I-ortboiodohippurate renography in 120patients whose urology inpatients.In accordance with the side of hydronephrosis,bilateral hydronephrosis and light,moderate and severe packet.The features of renography and the changes of functional parameter values in each group was analysised.And compared with the results of clinical evaluation about renal function and blood urea nitrogen(BUN),creatinine(Cr)levels.SPSS.16 statistical software was used for statistical analysis.Results The incidence of abnormal renal funetion in varying degrees of hydronephrosis were significantly higher than those without hydronephrosis(98.9% vs.39.3%,χ2=87.492,P<0.001).Presenting mainly high level and extension line and continued to inerease for the type of renography in mild to moderate degree hydronephrosis groups.Severe hydronephrosis is presenting mainly such as the low level and extension line for the type of renography.Varying degrees of hydronephrosis different have meant that the residual rate of increase of 15 minutes,both of the Enrichment rate and renal index(RI)is lower(F=74.56,20.0,61.917,P<0.001).The incidence of Renal insufficiency in the group of bilateral hydronephrosis was significantly higher than the unilateral hydronephrosis(55% vs 11%,χ2=13.848,P<0.001).And the extent of severe renal insuflciency,serum BUN and Cr levels were significantly higher than the group of unilateral hydronephrosis(F=13.671,24.302,P<0.001).The average of the functional parameters that both sides of renography there is a certain degree of correlation with BUN,Cr in the unilateral hydronephrosis group.Conclusion The 131I-orthoiodohippurate renography there is a higher sensitivity on the assessment of hydronephrosis and renal function,but lack of specificity.The analysis should be combination with clinical situation.

Abstract:
Objective To discuss 99Tcm-methoxyisobutylisonitrile(99Tcm-MIBI)imaging evaluation of the value of chemotherapy in patients with non-small cell lung cancer(NSCLC).Methods Seventy-one patients with NSCLC were studied with 99Tcm-MIBI SPECT before chemotherapy.The patients were classified by a follow-up CT as good responders(complete or partial remission)and poor respenders(stable disease or progressive disease).Following injection vein administration of 740MBq 99Tcm-MIBI,SPECT imaging at 10-30 min(early)and 2-3 h(delayed)were performed.Regions of interests were placed over the tumors and contralateral normal tissue on one transverse section.The 99Tcm-MIBI uptake ratio of the lesion to that in the contralateral normal lung was obtained from early images(ER)as well as delayed images(DR).The retention index(RI)was measured as:RI=(DR-ER)/ER×100%. 99Tcm-MIBI tumor uptake parameters were compared with the chemotherapeutic response.Results 99Tcm-MIBI uptake was significantly higher in responders than in non-responders:2.39±0.21 vs 1.89±0.19 and 2.50±0.19 vs 2.05±0.21 for ER and DR,respectively(t=8.311,8.480,P<0.05).The median of RI in good response group was significantly higher than that in poor response group(6.63% vs 5.13%,Z=2.416,P<0.05).Conclusion ER,DR and RI of 99Tcm-MIBI SPECT may benefit to evaluating chemotherapy response NSCLC.

Abstract:
Patient suffered from differentiated thyroid carcinoma(DTC)needs lifetime follow-up even after going through the thyroidectomy,postoperative 131I treatment,thyroid stimulating hormone suppressing therapy.There are disadvantages in DTC routine follow-up,such as conventional radiologieal studies,measurement of serum thyrogiobulin and 131I scintigraphy.It is not rare for us to see patients with dedifferentiated metastases which with no radioiodine uptake,and how to locate these dedifferentiated metastases is of great importance for making further treatment plan.Tumor imaging agent 99Tcm-methoxyisobutylisonitrile(99Tcm-MIBI)is widely used in the clinical and laboratories research of DTC in domestic and overseas,for its excellent physical and dosimetric characteristics and reasonable price.This review relates the application of 99Tcm-MIBI imaging in DTC follow-up,and focuses on its imaging mechanism,clinical methods,imaging interpretation,and the advantages.

Abstract:
While the diagnosis and treatment of diseases has entered the molecular level,apoptosis detection techniques are also continuously updated and developed,including magnetic resonance imaging,optical imaging,ultrasound and radionuclide imaging.Apoptosis plays a key role in the pathogenesis of many disorders including cerebral and myocardial ischemia,autoimmune neurodegenerative diseases,infections,organ and bone marrow transplant rejection,and tumor response to therapy.Several of these molecular steps have been evaluated to date,including phosphatidylserine exposure at the extracellular faces of the plasma membrane,detected by protein such as annexin V,caspase activation in the intracellular compartment,detected by labeled enzyme substrates or inhibitors,etc.Therefore,the physician could much better use this information to optimize the therapy.It is consistent with the characteristics of physiological metabolism.It is proven that apoptosis imaging in vivo imaging is early,non-invasive and quantitative.
Dynamically observing the value of the changes of serum sex hormone levels of early pregnancy after drug-induced abortion

Abstract:
Objective To observe the value of the changes of serum β-human chorionic gonadotropin(β-HCG),estradiol(E),progesterone(P)Levels of early pregnancy after drug-induced abortion dynamically.Methods Assessing 55 women proved pregnant by urine or blood HCG retrospecticly,who had terminated their pregnancy by mifepristonr and misoprostol.Meanwhile the serum levels of β-HCG,E,P were monitored dynamically.Results Among the 55 patients,the levels of β-HCG,E and P had significant decreased(tβ-HCG=4.845,tE=7.655,tP=11.390,P<0.01).On the 7th day,the levels of E and P decreased to the level of follicle phase.The level of E on the 14th day was higher than that of 7th day(tE=9.089,P<0.05); the level of P was higher than that of 14th day(tP=2.910,P<0.05).Conclusion Detecting the serum hormone's levels after drug-induced abortion by chemiluminescent immunoassay,we can assess indirectly the value of administration of mifepristone and misoprostol,predict the prolonged vaginal bleeding after drug-induced abortion,and the outcome of the treatment,which determine wether need another curestage.

Abstract:
Objective To assess the efficacy and safety of combined 99Tc-methylenediphosphonate(99Tc-MDP)and lower-dose methylprednisolone in the treatment of old patients with active moderate-to-severe Graves ophthalmopathy.Methods Twenty-four old patients with active moderate-to-severe Graves ophthalmopathy were randomly divided into two groups,that were methylprednisolone group(control group)and combination of 99Tc-MDP and methylprednisolone group(experimental group).The two groups were similar in age,sex,duration and severity of ophthahnopathy.Patients in control group were treated with intravenous high-dose methylprednisolone pulse therapy(cumulative dose of 800~1200 mg).experimental group were treated with combination therapy of 99Tc-MDP and lower-dose methylprednisolone(cumulative dose of 300~480 mg).At 12-week follow-up,therapeutic outcome was assessed by the change in clinical activity score and severity classified using Americom Thyroid Association of thyroid-associated ophthalmopathy indexing classification stcmdard.The adverse effects were observed and recorded in all patients.Results The remarkably effective outcome was observed in 6/12(50.0%)cases in control group and 10/12(83.3%)cases in experimental group.There were significant differences in the degree of improvements in ocular involvements.Less frequent and severe adverse effects were observed with the addition of 99Tc-MDP to lower-dose methylprednisolone therapy.Conclusion A combination of 99Tc-MDP and lower-dose methylprednisolone may be better tolerated and more effective than high-dose ophthalmopathy.
Efficacies of 125I seed implantation in advanced stage central lung cancer via fibrobronchoscope

Abstract:
Objective To explore the temporal curative effect of 125I seed implantation in advanced stage central type lung cancer.Methods 125I seed was implanted in 56 patients confirmed advanced stage central type lung cancer via fibrobronchoscope and all cases were fellow up in certain duration to explore their efficacies and the adverse reaction.Results Total efficient rate was 76.78% in 56 patients.Lung reexpanded rate was 90.90%.Conclusion The therapy of 125I seed implantation in advanced stage central type lung cancer is safe and available.

Abstract:
Objective In this study,we explored the potential clinical application of the instrument for automatic distribution of isotopes in order to reduce the injuries to operators by radiation.Methods The accuracy of samples and activities of isotopes were determined by acquisition of the samples through the instrument distributing multiple samples at different volumes.In addition,the therapeutical effects on the patients with thyroidism were compared between those who were treated by the isotope distributed via the instrument(IDI group)and those who were treated by the isotope distributed manually(IDM group).Results Acquisition errors by the instrument were under 0.1ml and the isotope distribution didn't affect the activity of radiation.Moreover,no significant difference was observed between the IDI group and the IDM group.Conclusion Isotope distribution by the instrument is accurate and reliable.And by this proceeding,radiation injuries to operators are decreased.

Abstract:
Objective To investigate and analyze the radiation dosage around the working environment in radiotherapy centre affiliated to Sichuan cancer hospital in the western China.Methods In 60 days,we have continuously monitored the accumulated dose that absorbed by doctors,nurses,technicians,physicists and engineers,and investigated the working environment 60Co unit,accelerator,after loading unit,X-ray simulator,CT simulator,gamma knife,MRI and doctor's office)and external environment by using TLD,and compared our results to those released by relevant departments.Results The average dosage in the working environment is 1.96 μC·kg-1·month-1,1.61 μC·kg-1·month-1 in external environment.Conclusion In the past 25 years,the radiotherapy center constructed strictly by the criterions of environment and protection departments required,so the radiation dosage in or outside the radiotherapy center has reached the national standard,which is safe for the staff and patients.Its instatement that the radiotherapy sites constructed by the related laws well accorded with the safety standards regulated.

Nuclear cardiology,cardiac CT scanner and coronary angiography are common used in patients with coronary artery disease.Cardiac diagnostic imaging play important role with decreasing the incidence and morbidity of coronary artery disease.Radiation dose are concerned by physicians and the public due to the development of cardiac diagnostic imaging.Through evaluating and minimizing the radiation dose,cardiac diagnostic imaging can be used in clinical practice widely and reasonably.

Abstract:
Objective To research radiation protective nursing intervene and important notice of 125I seeds minimally invasive implantation in non-small cell lung carcinoma(NSCLC)by CT.Methods Under the system of therapy planning system(TPS)and posologic validation, 125I seeds were implanted in 89 cases of NSCLC patients.The consistent radiation protective nursing intervene was used in perioperative period management.The operative successful rate,therapeutic effect and complication rate was observed.Results The scientific radiation protective nursing intervene can ensure that the radioactive dose distribution of 125I seed implantation brachytherapy is consistent with the principles of effective and minimally invasive.The operative successful rate was 100%.The local control rate and 1 year survival rate respectively was 97.4%and 92.2%.But the early and later incidence rate of radioactive damaging effect was 14.6% and 1.1%respectively.Leakage of radioactive contamination has not occurred.Conclusion The consistent TPS and posologic validation 125I seeds implantation integrated scientific radiation protective nursing intervene.It is very important to improve the therapeutic effect of NSCLC and reduce the incidence of complications.
The value of 16 slice spiral CT detecting tracheobronchial tumors

Abstract:
Objective To discuss the value of 16 slice spiral CT detecting tracheobronchial tumors.Methods Twenty-eight cases with suspected tracheobronchial principalis tumors undertook GE Light speed16 CT scan.The original data were processed and reconstructed at AW4.2 workstation.The images of trachea and bronchial tumors were reviewed and compared with pathologic results after operation.Results Out of the 28 cases with tracheobronchus principalis tumors,the CT detected 16 with trachea tumors and 12 with bronchial tumors(6 on the fight and 6 on the left).Among them,23 were squamous carcinoma with 2adenocarcinoma,1 leiomyoma,1 adenoma and 1 adenoid cystic carcinoma,which were consistent with pathologic results after operation.Conclusion The 16 slice spiral CT scan combining reconstruction techniques can detect the location of the tracheobronchial tumors and make qualitation and quantitation diagnosis,which is an accurate and noninvasive diagnosis method.It is important in clinical diagnosis.
