近年来,18F-FDG PET、PET-CT已广泛应用于甲状腺癌术后复发和(或)转移灶探测及预后评估中,但对于甲状腺结节术前的诊断作用仍不清楚。相比超声、CT等传统影像学手段,18F-FDG PET、PET-CT尚未作为甲状腺癌术前的常规检查项目,一是由于其价格相对昂贵,二是由于仍缺少大量研究证实PET、PET-CT可提供额外信息并改变原来的手术方式。18F-FDG的标准化摄取值(standardized uptake value,SUV)是PET、PET-CT最常用的半定量分析指标,通过描述组织葡萄糖代谢,用于鉴别肿瘤良恶性。对于细针穿刺后性质仍不明确的甲状腺结节,SUV能否鉴别其良恶性?对于因其他肿瘤行PET、PET-CT而意外发现的甲状腺结节,SUV能否鉴别其良恶性?SUV对于甲状腺结节的诊断有多少意义?本文就以上三方面对SUV在鉴别甲状腺结节良恶性方面的诊断价值进行综述。

甲状腺癌最常见的临床表现为甲状腺结节,呈单发结节或呈多发结节肿块,其中约5%~10%可扪及,17%~45%由超声发现。目前超声引导下的细针穿刺是甲状腺结节的首要评估手段,其准确率达70%~97%[1]。但仍有16%~24%的甲状腺单发结节经细针穿刺后仍不能确诊,其中病理类型通常包括:滤泡状癌、Hurthele细胞癌、非典型细胞等。这些患者往往需行甲状腺腺叶切除后才能确诊,但其中仅20%~30%病理诊断为恶性,所以有超过60%~70%的患者接受了不必要的甲状腺切除手术。目前并没有制定针对细针穿刺后性质仍不明确的甲状腺结节处理方案,超声、CT和MRI已证实存在一定的诊断价值,而PET、PET-CT仍需要一个大样本的前瞻性研究来证明其价值[2]。表 1列举了PET、PET-CT在细针穿刺后性质仍不明确的甲状腺结节中的研究结果。

由表 1可见,PET-CT的灵敏度较高,即对恶性结节的检出能力较高,漏诊率低;其阴性预测值也较高,即对PET阴性的甲状腺结节基本可排除其恶性可能。PET-CT对甲状腺结节诊断的灵敏度、特异度、阳性预测值、阴性预测值及准确率与PET阳性标准的选择直接相关,因此一些研究建议对SUV进行校正,找出适合的SUVmax分界点以达到较高的灵敏度。
Deandreis等[1]将PET-CT和B超检查进行比较,对56例细针穿刺后性质仍不明确的甲状腺结节患者于术前分别行B超检查及PET-CT,术后病理显示:共有34例良性结节、10例恶性结节(7例乳头状癌、3例滤泡状癌)及12例恶性潜能不明的结节,结果:B超检查与PET-CT的灵敏度、特异度、阳性预测值、阴性预测值及准确率分别为82%和77%、47%和62%、50%和57%、80%和81%,61%和68%。由此可见,PET-CT与B超检查相比,其在甲状腺结节良恶性的诊断上并无显著优势。
Vriens等[7]对于术前接受PET-CT的225例细针穿刺不明确的甲状腺结节患者进行回顾性的meta分析,其灵敏度、特异度、阳性预测值、阴性预测值及准确率分别为95%,48%,39%,96%及60%;结论:若甲状腺结节>15 mm且PET-CT为阴性,则可以排除甲状腺癌;PET诊断为阳性的甲状腺结节使其恶性危险度由25.8%提高至38.7%,但由于PET-CT阳性预测值不高,PET阳性并不能鉴别其良恶性,因为其中约50%的结节为良性,所以这些患者仍需要进一步检查。因此,对于细针穿刺后仍不能确定性质的甲状腺结节,PET-CT不能鉴别其良恶性。

随着PET-CT在肿瘤筛查、分级及治疗疗效评估中的应用逐渐被推广和甲状腺癌发病率的逐渐提高,PET-CT意外发现甲状腺18F-FDG代谢增高的概率为1%~3%,通常表现为甲状腺局部18F-FDG代谢增高灶及弥漫性18F-FDG代谢增高灶。甲状腺弥漫性18F-FDG代谢增高灶与自身免疫性甲状腺炎有关,尤其是桥本甲状腺炎,而甲状腺局部18F-FDG代谢增高灶提示为腺瘤或甲状腺癌[8]。Ohba等[9]报道,因非甲状腺恶性肿瘤行PET-CT后意外发现甲状腺癌的概率为30%~50%,因健康体检行PET-CT后意外发现甲状腺癌的概率为6.7%~32.9%。
许多研究对SUV能否鉴别甲状腺良恶性结节进行了大样本回顾性分析,所收录的患者因各类肿瘤筛查、分期及治疗后再分期而行PET-CT,并且意外发现甲状腺18F-FDG代谢增高灶。通过分析甲状腺意外18F-FDG摄取灶的发生率、恶性率,以及良恶性结节的SUVmax的差异性,可绘制受试者曲线以找到良恶性结节SUVmax分界点,还可以指导临床的进一步检查或治疗方案的制定。表 2例举了PET、PET-CT在意外发现的甲状腺结节中的研究结果。

由表 2可见,意外发现的甲状腺18F-FDG阳性病灶的发生率为1.0%~3.7%,结节的恶性发生率为14%~43%。大部分研究结果显示,SUV并不能鉴别甲状腺结节的良恶性,所以仍需进一步的超声引导下细针穿刺及细胞学检查,然而Boeckmann等[14]的研究提示,SUVmax=4.2可作为鉴别良恶性结节的分界点;且另有研究提示,良恶性结节的SUVmax分界点在3.5~5.0[15-17]。虽然各研究设计不同,但各个SUVmax分界点较接近,仍有借鉴价值。Boeckmann等对术前行多次PET-CT的患者进行了研究分析发现,2年内甲状腺结节的SUV相对稳定,其变化不可提示甲状腺结节的良恶性。
也有文献对甲状腺结节延迟PET-CT的SUV进行了研究,以求找到新的SUV鉴别甲状腺良恶性的信息。D′Souza等[18]做了一项前瞻性的大样本随机对照试验,对200例意外发现甲状腺结节的患者行术前B超检查、PET-CT(60 min)及延迟PET-CT(120 min),并跟踪记录术后病理结果,结果显示,PET-CT延迟显像与PET-CT相比,SUVmax升高10%,恶性结节的诊断灵敏度、特异度由80.8%和84.5%提高至84.6%和85.6%;从PET-CT(60 min)的受试者工作特征曲线中得出SUVmax为8.1时灵敏度、特异度最高(80.77%和84.48%)。但PET-CT延迟显像与B超检查在灵敏度、特异度、阳性预测值、阴性预测值间的比较都无明显差异。
综上可见,SUV在鉴别甲状腺结节良恶性上的作用有限,笔者分析原因如下:
(1)细胞分化程度是预测18F-FDG摄取量的独立因素[1],甲状腺乳头状癌由于受到局部容积效应、细胞分化程度等因素的影响,在PET-CT上可能为阴性,导致术前PET-CT出现假阴性[19];滤泡状癌在性质不明结节中恶性比率占10%~20%,其SUV比其他组织类型的甲状腺癌都低。这些现象的存在降低了PET-CT鉴别良恶性结节的价值。
(2)通常认为病灶有18F-FDG摄取即为18F-FDG PET阳性,无18F-FDG摄取为18F-FDG PET阴性。但这样的标准使得PET-CT的假阳性率升高,一些研究建议将局部SUV摄取>2.0作为PET阳性的标准[6]。但无论采用什么标准,假阳性和假阴性之间总是存在平衡问题。
(3)纳入和剔除患者的标准不同、采用不同的参数及统计学方法。
(4)细针穿刺诊断不明确的甲状腺结节在术后病理诊断中也存在困难。
(5)SUV与患者体重、18F-FDG摄取时间、患者血糖水平及部分容积效应有关,这些因素未进行控制,或未对18F-FDG进行校正,都可能给统计结果带来误差。
所以目前认为,SUV在鉴别甲状腺结节良恶性方面的作用是有限的。甲状腺发生意外的局灶性18F-FDG摄取的概率较低,但由于其存在一定程度的恶性发生率,仍需行细针穿刺进行进一步评估。在鉴别甲状腺结节良恶性方面,PET-CT并不优于B超检查,但其优势在于对甲状腺局部播散及远处转移灶的探测。

Abstract: 18F-FDG PET and PET-CT has become an important tool in the postoperative management of de-differentiated thyroid cancer. But the value of this method in the preoperative diagnosis of thyroid nodules is unclear. Various groups of investigators have explored the potential role of 18F-FDG PET and PET-CT in patients with benign and malignant thyroid neoplasms expecially those with indeterminate fine needle aspiration cytology findings. With the increasing demand for 18F-FDG PET and PET-CT, clinicians are faced with the challenge of managing an increasing number of 18F-FDG PET and PET-CT detected thyroid incidentalomas because their significance remains unclear. The standardized uptake value of 18F-FDG is the most common semi-quantitive index and can be used to characterize malignant and benign thyroid nodules by describing the metablism of glucose. This article reviewed the value of SUV in differential diagnosis of malignant and benign thyroid nodules. -
