放化疗已成为大部分不能手术切除的局部进展晚期肺癌、食道癌等胸部肿瘤的一种治疗选择。与单纯放疗相比, 这种综合治疗提高了肿瘤的局部控制率和患者生存率。然而, 这种综合治疗所付出的代价是增加了正常肺组织毒性反应的发生率和严重性。放疗和化疗引起的治疗相关性肺损伤的发生率为13%~44%。发生率的不同与采用不同的标准、入组研究的患者不均一、治疗方法不同以及放疗技术不同等因素相关[1-3]。比如肺癌放化疗引起的3、4级放射性肺炎和肺纤维化发生率为25%~30%[4-5], 而且在某些情况下可能是致死性的。放化疗相关性肺损伤已成为限制放化疗效果的主要因素之一, 影响了患者的生存质量, 甚至危及生命, 故及时正确的诊断及处理至关重要。本文综述了近年来国内外对放化疗相关性肺损伤所进行的一系列相关研究。

摘要: 放化疗已成为大部分不能手术切除的局部进展晚期肺癌、食道癌等胸部肿瘤的主要治疗选择。而放化疗引起的治疗相关性肺损伤成为限制治疗效果的主要因素之一,同时也影响了患者的生存质量,甚至危及生命。该文综述了近年来放化疗相关性肺损伤在发病机制、病理改变、影像学表现及诊治方面的研究现状和进展。Abstract: Chemoradiotherapy is the main treatment option for most locally advanced unrespectable lung cancer, esophageal cancer and other chest cancer.But cure-related lung injury becomes one of the main constraint factors, and also affects the patients'quality of life, even threatening their lives.This paper reviews the progress of research on chemoradio therapy-related lung injury in the pathogenesis, pathology, imaging and treatment.
