恶性肿瘤患者会发生骨转移, 乳腺癌和前列腺癌在各种肿瘤骨转移病例中占80%以上[1]。65%左右的骨转移患者是有症状的, 70%~80%以首发骨痛就诊而发现骨转移, 骨转移可以导致骨相关事件(skeletal-related events), 包括病理性骨折、脊髓压缩、高钙血症、需要放射治疗和矫形外科缓解的骨痛等[2], 严重影响患者的生存质量。早期发现和治疗骨转移性骨痛可以预防骨相关事件的发生, 改善患者的生存质量。

骨转移破坏了正常骨代谢动态平衡, 破骨细胞活性增加, 溶骨作用增强, 钙从骨基质释放入血, 形成高钙血症, 骨皮质变薄易形成病理性骨折, 多种细胞因子作用刺激神经末梢时可产生骨痛[3]。其主要机制包括: 肿瘤细胞侵袭和从原位肿瘤逃逸; 肿瘤细胞靶向迁移至骨; 特殊黏附与骨结合; 肿瘤细胞通过与骨微环境相互作用在骨中存活增殖。肿瘤骨转移有成骨性、溶骨性和混合型转移, 大部分骨转移以溶骨性病变为主, 15%~20%以成骨性病变为主[4]。用于治疗骨转移的放射性核素具有趋骨性, 其衰变过程中产生的辐射能破坏骨转移瘤细胞而发挥治疗作用。

用于治疗转移性骨痛的放射性核素主要有5大类: 89Sr、153Sm、186Re/188Re、117Sn、32P。用这些核素标记的常用转移性骨痛药物及其基本参数对照见表 1。

89SrCl2、153Sm-乙二胺四亚甲基膦酸(153Sm-ethylene diamine tetramethylene phosphonic acid, 153Sm-EDTMP)、186Re-羟基亚乙基二膦酸(186Re-hydroxy ethylidene diphosphonic acid, 186Re-HEDP)已被不同国家批准用于治疗成骨性或混合型乳腺癌和前列腺癌转移性骨痛, 以及在治疗前至少8周的全身骨显像证实的任何其他肿瘤的疼痛性成骨病变[5]。很多患者往往在化疗、放疗或其他药物治疗失败时才选择使用放射性核素治疗, 把它作为最终治疗手段, 这其实是个误区。尽早使用放射性核素治疗成骨性骨转移性骨痛在很多临床研究中已被证实是一种安全的治疗方法[6], 并且仍可继续进行全身化疗或放疗。

放射性核素治疗转移性骨痛的绝对禁忌证是怀孕和哺乳, 接受单次治疗剂量后的患者应至少避孕6~12个月。相对禁忌证有肾小球滤过率 < 30 ml/ min或血透; X线证实的脊髓压缩性骨折; 广泛骨髓受累(如全血细胞减少或“超级显像”)。
治疗前充分准确评估患者情况将会使放射性损害降到最低, 这些评估包括: 治疗前8周内的骨扫描(阳性结果: 成骨性病变和疼痛部位一致); 治疗前4~12周无化疗或广泛放疗, 血常规检查(血红蛋白 > 90 g/L, 白细胞数 > 3.5×109/L, 嗜中性粒细胞数 > 1.5×109/L, 血小板数 > 100×109/L)等。

放射性核素治疗转移性骨痛的效果是肯定的, 其疗效主要根据患者的疼痛缓解度、麻醉镇痛药量、总生存期、不良反应和生存质量来评价[7]。Tripathi等[8]报道, 86例转移性骨痛患者(包括乳腺癌、前列腺癌和其他肿瘤)使用37 MBq/kg 153Sm-EDTMP的疼痛缓解率为73%。Bauman等[7]对20个I~III期的放射性核素治疗转移性骨痛随机试验进行了系统回顾, 受试者中乳腺癌、前列腺癌和肺癌的百分比分别为5%~10%、80%~90%和5%~10%, 涉及的放射性核素包括89Sr、153Sm、186Re/188Re、117Sn、32P、117Tn, 结果: 89Sr、153Sm、186Re能缓解疼痛和减少镇痛药物的使用, 完全缓解率为20%~30%, 但对延长患者生存期无明显效果, 主要不良反应为轻中度的血细胞减少症, 恶心和呕吐的发生率 < 10%, 明显低于半身(43%)或局部放疗(27%), 作者的结论是对于多发性转移性骨痛用镇痛药物疗效不佳且有骨扫描证实病灶的患者, 可选择单种放射性核素(89Sr、153Sm)缓解疼痛。
放射性核素自身存在差异, 那么这种差异对治疗效果是否有影响呢?Baczyk等[9]将100例转移性骨痛患者(原发灶分别为乳腺癌40例, 女性; 前列腺癌60例, 男性)分成两组, 每组50例, 分别给予150 MBq 89Sr和37 MBq/kg 153Sm, 结果: 153Sm组疼痛完全缓解率女性和男性均为40%, 89Sr组的女性和男性分别为25%和33%, 共有20%的患者无效; 对成骨性转移使用视觉模拟评分系统评估89Sr和153Sm治疗的疼痛缓解完全有效率分别为42.9%和48.6%, 对混合型转移的完全有效率分别为0%和20%, 153Sm的治疗效果似乎优于89Sr。Liepe等[10]观察了15例乳腺癌和前列腺癌转移性骨痛患者使用148 MBq 89SrCl2、37 MBq/kg 153Sm-EDTMP和1295 MBq 186Re-HEDP治疗的疼痛缓解率, 结果分别为72%、73%和67%。

摘要: 恶性肿瘤会发生骨转移,早期发现骨转移可以预防骨相关事件的发生,改善预后。用于治疗转移性骨痛的放射性核素有5大类:89Sr、153Sm、186Re/188Re、117Sn、32P。放射性药物治疗转移性骨痛的效果较好,尤其适用于成骨性或混合型骨转移。放射性药物联合双磷酸盐药物治疗广泛转移性骨痛效果显著,优于两者的单独使用,明显改善患者生存质量。Abstract: Malignant tumors presented with bone metastasis.Earlier detection of bone metastasis can prevent skeletal-related events and improve prognosis.Common radiopharmaceuticals for bone metastasis include 89Sr, 153Sm, 186Re/188Re, 117Sn and 32P, the effectiveness of them is identified, especially for osteoplastic metastasis and mixed metastasis.Radiopharmaceuticals combined with bisphosphonates for extensive painful bone metastasis is significantly more effective than using either of them alone, in improving the life quality of cancer patients.
