甲状腺功能亢进症(简称甲亢)是多种病因导致的体内甲状腺激素分泌过多,以全身多系统兴奋性增高和代谢亢进为主要表现的疾病的总称[1]。临床上以格雷夫斯病(Graves' disease,GD)最为常见(约占85%)[2]。GD是一种自身免疫性疾病,是患者体内TSH受体抗体(thyrotropin receptor antibodies,TRAb)刺激甲状腺细胞的TSH受体,导致甲状腺激素合成分泌增多所致,其发病率为0.2%~0.5%[3]。GD的治疗方法包括抗甲状腺药物、手术和131I治疗,3种方法各有利弊。131I治疗较前2种方法具有疗效好、不良反应少、起效迅速的优势,但131I治疗剂量的选择仍存在争议。131I治疗剂量的选择主要基于2个不同的治疗目标:一种是尝试给予患者足够的剂量,使患者恢复正常的甲状腺功能,但不造成甲状腺功能减退(简称甲减);另一种是为避免复发,直接诱发患者甲减,随后进行药物替代治疗。国内学者多选择前者作为治疗目标,国外学者尤其是美国学者多选择后者作为治疗目标,这是131I治疗剂量选择不一致的主要原因。目前常用的确定131I治疗剂量的方法包括计算(标准)剂量法、固定剂量法、半固定剂量法、消融剂量法和小剂量法等[1],现将常用的131I治疗剂量的确定方法进行综述。

计算(标准)剂量法主要包括3种不同的计算方式:以每克甲状腺组织实际摄取的放射性活度为依据计算131I的治疗剂量;以甲状腺目标吸收剂量为依据计算131I的治疗剂量;以甲状腺最终质量的预测模型为依据计算131I的治疗剂量。

以每克甲状腺组织实际摄取的放射性活度为依据计算131I的治疗剂量的具体计算公式如下:

公式(1)是我国最常用的计算公式,祝林等[4]运用该公式计算得到的131I治疗剂量治疗了497例甲亢患者,随访1年后发现,患者的治疗成功(包括甲状腺功能正常及甲减)率为93.9%。公式(1)中甲状腺的质量尚无简单、精确的估算方法,常用超声法与触诊法相结合进行评估。Chen等[5]的研究结果显示,口服131I放射性活度的最佳范围为1.85~4.07 MBq,患者经131I治疗后,甲状腺功能正常者为71.8%。我国的专家共识推荐口服131I放射性活度的范围为2.59~4.44 MBq(70~120 μCi)[1]。美国甲状腺协会(ATA)指南推荐的131I放射性活度范围为3~8 MBq(80~220 μCi)[6],国内外对该范围的设定较宽泛,主要取决于不同的治疗目标。若治疗目标为使患者甲减,选择较高的131I放射性活度可以提高单次治疗成功率。马玉波等[7]在公式(1)的基础上采用甲状腺静态显像放射性单位时间活度下的总计数(ROI)比值计算131I的放射性活度,公式如下:

马玉波等[7]运用公式(2)计算的131I治疗剂量治疗了95例甲亢患者,并与通过公式(1)计算的131I治疗剂量治疗的98例甲亢患者进行比较,随访1年后发现,两组患者治疗成功率的差异无统计学意义,但运用公式(2)计算的131I治疗剂量治疗的患者的甲减发生率更低(8.4%),且差异有统计学意义(P<0.05)。在不影响疗效的前提下,降低甲减发生率是我们追求的目标。但该研究的样本量较小,还需更多研究证明该公式的有效性。

甲状腺目标吸收剂量(absorbed dose,Da)是指患者口服131I后,单位甲状腺组织吸收的平均辐射剂量的大小。Marinelli LD公式[8]从1948年沿用至今,在国外的使用较为广泛。依据所设定的不同的目标吸收剂量,在Marinelli LD公式[8]的基础上衍生出许多不同的公式(表1)。

国内多采用以下公式[13]:

Wei等[14]利用公式(3)计算的131I治疗剂量治疗了766例甲亢患者,随访1年后发现,患者的治疗成功率为78.7%,同时,研究人员发现,甲状腺最高摄131I率越高,有效半衰期越长,单次治疗成功率也越高。
患者的Da会影响预后:当Da<20 Gy时,单次治疗成功率(包括甲状腺功能正常和甲减)<20%[15];当Da=50 Gy时,单次治疗成功率为33%[14];当Da=100 Gy时,单次治疗成功率为78.7%[11];当Da在200~250 Gy时,单次治疗成功率可达89%[16];当Da=300 Gy时,随访1年的单次治疗成功率为100%[17]。Willegaignon等[18]对最佳Da进行研究,结果发现,若单次治疗成功率>95%,Da应≥200 Gy,这与Schiavo等[19]的观点一致。

Malapure等[20]研究发现,131I治疗后,甲亢未缓解者甲状腺体积的改变小于甲状腺功能正常或甲减者。131I的治疗剂量与甲状腺体积改变之间的相关性研究结果表明,通过预测治疗后甲状腺的最终体积,可以在治疗前估测最佳的131I剂量。该模型的建立基于一个假设:甲状腺体积的减少是由于甲状腺细胞失活而存在剂量依赖性。基于上述假设,对电离辐射下的甲状腺存活细胞进行数学描述,得到131I治疗后甲状腺最终质量的预测模型[21]:

$ \Delta $ np=0.032 mGy·g·MBq−1·s−1;U=Am/A0;Am为甲状腺最大放射性活度(MBq);A0为口服131I的放射性活度(MBq);Tmax为最大摄取时间(h);γ=2.2×10−6;Teff为131I有效半衰期(h)。Traino等[22]运用公式(4)对22例甲亢患者131I治疗后甲状腺的最终质量进行预测,并将预测结果与超声测量的甲状腺体积进行相关性分析,发现二者具有较强的相关性(r=0.81)。在此基础上,得到关于131I治疗剂量的预测公式:

$ \Delta $ np=0.032 mGy·g·MBq−1·s−1;U=Am/A0;Am为甲状腺最大放射性活度(MBq);mfin为131I治疗后甲状腺的最终质量(g);Teff为131I有效半衰期(h)。Traino等[22]运用公式(5)对40例GD患者进行131I治疗剂量的预测,并与设置目标剂量的计算方法相比较,结果显示,前者的平均剂量为394 MBq,后者的中位剂量为529 MBq。研究者认为,在相同的疗效下,前者的剂量更少,可能存在一定的优势。但这个模型是基于一个非常理想化的假设:131I在甲状腺内均匀分布。因此该模型仅为临床剂量的制定提供了新的思路,还需要进一步的优化。

固定剂量法即给予患者一个固定的口服131I的剂量。最初使用固定剂量法时,其剂量范围为111~185 MBq(3~5 mCi),但单次治疗成功率只能达到60%。Aktaş等[23]比较多种131I固定剂量(102、136、204、272、340 MBq)的疗效,认为131I的最佳剂量应≥340 MBq,并依据患者的情况适当增加剂量。Schiavo等[19]对多种131I固定剂量范围(350~399 MBq、40~499 MBq、500~599 MBq、≥600 MBq)的疗效进行了比较,认为最佳剂量范围应为500~599 MBq。Jaiswal 等[24]对使用340、555 MBq两种131I固定剂量进行治疗的患者的疗效进行比较,发现两组患者的疗效无明显差别,但后者的甲减发生率更高,这与Santos等[25]的研究结果一致。目前欧洲甲状腺协会Graves甲状腺功能亢进治疗指南[26]推荐的131I固定剂量范围为185~555 MBq(5~15 mCi),2016年美国甲状腺协会(ATA)发布的《甲状腺功能亢进和其他原因导致的甲状腺毒症诊断和管理指南》 [6]和我国专家共识[1]推荐的范围为370~740 MBq(10~20 mCi),产生差异的原因可能是欧洲国家对辐射剂量的管理非常严格,多采用较低剂量的131I进行治疗,治疗后复发率较高,因此131I不作为欧洲国家治疗GD的首选方案。
半固定剂量法即在固定剂量的基础上,依据不同的甲状腺质量,给予甲亢患者固定剂量的131I。我国的专家共识推荐半固定剂量法的具体方案为:较小甲状腺(<30 g)剂量为 185 MBq,中等大小甲状腺(30~50 g)剂量为 370 MBq,较大甲状腺(>50 g)剂量为555 MBq[1]。

消融剂量法即患者单次口服大剂量131I。固定剂量方案多采用低剂量(555 MBq)或高剂量(1110 MBq)进行甲状腺消融;若通过计算剂量方案来确定剂量,目标吸收剂量多设置为300 Gy或每克甲状腺组织5.10~10.20 MBq。Sapienza等 [27]对比分析了555 MBq和1110 MBq 131I治疗GD的疗效,结果表明,单次治疗成功率分别为70%和97%;同时研究人员运用医疗内辐射剂量委员会(Medical Internal Radiation Dose Committee,MIRD)提出的计算吸收剂量的方法(简称为MIRD法)[28]计算目标吸收剂量,对比接受≤300 Gy和>300 Gy131I治疗剂量的GD患者的疗效,结果显示,单次治疗成功率分别为63%和89%(P=0.01)。该方法的治疗目标为一次性治疗GD,为了确保单次治疗成功率,避免复发,给予患者的131I剂量会在某些情况下超过患者实际所需要的剂量。因此该方法多适用于重症甲亢患者。
2020年的一项研究结果显示,采用1110 MBq的131I固定剂量方案治疗非毒性多发结节性甲状腺肿的疗效优于手术治疗[29]。Ariamanesh 等[30]研究发现,对于伴有Graves眼病的GD患者,555 MBq固定剂量法可以减缓眼病的恶化并改善症状,其可能的机制是永久性去除甲亢后,甲状腺和眼交叉反应抗原也被去除,消除了甲状腺自身免疫淋巴细胞的来源。Wiersinga [31]则认为,当甲状腺被131I破坏后,甲状腺自身抗体的释放可能会造成二次免疫,使Graves眼病加重,因此对合并Graves眼病的GD患者行131I治疗时应慎重。

小剂量法主要包括两种方案,一种是口服固定小剂量的131I,口服剂量为17~85 MBq,间隔一定时间复查甲状腺功能,若显示甲亢未缓解则再次口服,直至症状缓解;另一种是仅口服常规剂量的一部分。Lowdell 等[32]使用131I固定小剂量74 MBq(2 mCi)对164例甲亢患者进行治疗,所有患者在治疗后6周复查甲状腺功能,以后每3个月复查一次,对于甲亢未缓解的患者,1年后再次口服小剂量131I,平均随访4.5年后发现患者的甲亢未缓解率为2%,甲减发生率为18%。唐晓燕等[33]对行多次小剂量131I治疗的325例甲亢患者的疗效进行评估,患者每次口服常规剂量的1/2~2/3(总剂量为1.5~8 mCi),3个月为1个疗程,直至治愈。结果显示,患者的治疗成功率为94.5%,其中仅4例患者出现了早发甲减。由此可见,该方法可以减少早发甲减的发生,但晚发甲减的发生率并未明显减少[34],且该方法治疗时间较长,因此未被临床推广使用。

王舰等[35]对使用固定剂量法(185 MBq)和公式(1)计算的131I治疗剂量进行治疗的患者的疗效进行评估,结果显示,使用两种治疗剂量的患者治疗成功率的差异无统计学意义,但使用固定剂量法的患者的甲减发生率较高,这与Traino等[21]的观点一致。Hyer等[9]研究发现,计算剂量法较固定剂量法在GD患者中的偏差小,使用Marinelli LD公式[8]的计算剂量法的偏差最小。其主要原因是固定剂量法没有考虑患者的个体化因素,例如甲状腺质量、摄碘率和有效半衰期等。Pardo等[36]将半固定剂量法与计算剂量法进行比较,结果显示,后者的单次治疗成功率更高、甲减发生率更低(P<0.05)。Haase等[37]将半固定剂量法与固定剂量法进行比较,发现对于甲状腺质量相同的患者,采用半固定剂量法的单次治疗成功率较高、同时甲减的发生率较低(P<0.05)。 不同131I剂量确定方法之间优缺点的比较见表2。

摘要: 131I 治疗格雷夫斯病(GD)的疗效和安全性等已得到国内外学者认可,但对具体治疗剂量的选择仍然存在一定争议。治疗目标不同,剂量选择方案也不同。国内学者多选择计算剂量法与个体化相结合的方案,国外学者多选择固定剂量方案。笔者就国内外131I 治疗GD的剂量确定方法作一综述。Abstract: The curative efficacy and safety of the 131I therapy of Graves' disease have been recognized by most domestic and international scholars, but the choice of the specific therapeutic dose is still controversial. According to different therapeutic targets, the choice of the dose-determination scheme is also different. Domestic scholars mostly choose the combination of calculated dose method individualized scheme, while foreign scholars mostly choose the fixed dose scheme. This article reviews the dose-determination schemes of 131I therapy for Graves' disease.
