前列腺癌是最常见的威胁男性健康的肿瘤之一,近年来其发病率逐渐上升[1]。中国国家癌症中心的数据显示,在2015年中国男性恶性肿瘤中,前列腺癌的发病率已上升至第六位[2]。目前,常规的诊疗方法如手术和药物去势治疗等已显著提高了前列腺癌患者的生存质量,然而,术后生化复发以及转移性去势抵抗性前列腺癌(metastatic castration resistant prostate cancer,mCRPC)患者的预后依然不佳。在前列腺癌中,前列腺特异膜抗原(prostate-specific membrane antigen,PSMA)的特异性过表达水平明显高于前列腺特异性抗原(prostate-specific antigen,PSA)。采用放射性核素标记与PSMA有高亲和力的小分子抑制剂,从而实现对前列腺癌及其转移灶显像和治疗的方法已取得了显著的效果,尤其是应用68Ga标记的PSMA小分子抑制剂进行PET/CT显像已经应用于临床。近年来,PSMA小分子抑制剂的单光子显像在前列腺癌的诊疗中也取得了一定进展,但相应的报道不多。因此,我们对单光子核素标记的PSMA小分子抑制剂在前列腺癌中的临床应用进展进行综述。

目前,常用于标记化合物的单光子核素包括99Tcm、123I、125I、131I和111In。其中,99Tcm是单光子放射性药物领域使用最多的核素。作为99Tc的核同质异能素,99Tcm具有半衰期短且无其他粒子辐射等优点,其标记的化合物对患者的辐射剂量较小,适合于放射性药物的制备和临床显像。99Tcm标记的药物已广泛应用于甲状腺、心肌、骨相关疾病以及肿瘤的临床显像诊断与脏器功能的评估。123I和125I是碘的放射性同位素。123I的半衰期相对较短(13.2 h),可发射159 keV能量,且衰变时不会释放β粒子,其标记的显像剂可应用于心脏交感神经受体的临床显像[16-17]。125I的半衰期长且能量较低,应用于SPECT显像诊断并无优势,目前主要应用于体外放射性免疫分析以及粒子植入治疗肿瘤等方面。131I是一种人工放射性核素,半衰期为8.3 d,发射β射线(99%)和γ射线(1%),因其能在人体甲状腺中浓聚而广泛应用于甲状腺疾病的显像和治疗。111In也是一种人工放射性核素,其半衰期较长(2.83 d),以电子俘获形式进行衰变,同时释放出能量为172 keV和245 keV的γ射线。研究者已开发出许多111In标记的SPECT显像探针,但目前受到更多关注的是111In在指导放射性导向手术(radiation-guided surgery,RGS)中的巨大潜力[18-19]。
PSMA小分子抑制剂主要分为三类:磷酸盐化合物、巯基类化合物和尿素衍生物[20]。其中,尿素衍生物在临床前研究和临床应用中最为多见,单光子核素标记的巯基类化合物尚未见临床方面的相关报道。表1对部分应用于动物和人体的单光子核素标记的PMSA小分子抑制剂进行了总结。

磷酸盐类化合物对PSMA阳性肿瘤具有很高的亲和力和特异性,正电子核素标记的小分子抑制剂,如64Cu-CBT2G和18F-CTT1298等在临床前研究中显示出了令人满意的显像效果[35-36]。然而,磷酸基团极性高、代谢慢等不足限制了此类PSMA小分子抑制剂的应用。
Nedrow-Byers 等[21]以DTPA为螯合剂,基于磷脂类的不可逆抑制剂(2S)-7-氨基-2-((((((S)-1,3-二羧基丙基)氨基)(羟基)磷酰基)氧基)甲基)-4-氧代辛二酸(CTT-54)合成了SPECT显像剂99Tcm-(CO)3-DTPA-CTT-54(目前单光子核素标记的磷酸盐类衍生物仅此一种)并应用于小鼠体内进行了初步的显像研究。结果发现,99Tcm-(CO)3-DTPA-CTT-54能快速内化到人前列腺癌LNCaP细胞中,其在PSMA阳性表达的LNCaP细胞中表现出时间依赖性的摄取,并且可在非靶组织中被快速清除。这为研发下一代磷酸盐类SPECT显像剂提供了理论支持。

巯基类衍生物如经典抑制剂2-(3-巯丙基)戊二酸(2-(3-mercaptopropyl) pentanedioic acid,2-MPPA)具有更好的膜通透性和更高的口服生物利用度,因此,其曾被认为是磷酸盐类化合物的一种很好的替代品。但巯基类化合物与PSMA的结合无特异性,同时,硫醇基团的亲核性质使其容易氧化,当其与内源蛋白形成偶联时,有损害代谢稳定性和诱导免疫反应的风险,这极大限制了其在临床上的应用[37]。目前尚未有应用正电子核素或者单光子核素对此类化合物进行标记的文献报道。

尿素类PSMA小分子抑制剂是含有两个氨基酸(如半胱氨酸-尿素-谷氨酸、谷氨酸-尿素-赖氨酸)形成脲键(-NH-CO-NH-)的N-乙酰天冬氨酰谷氨酸类似物,其可被PSMA水解成N-乙酰天冬氨酸和谷氨酸。此类小分子抑制剂对PSMA具有高亲和力和特异性,且一旦结合于活性位点,其可迅速内化到细胞中[38]。此类化合物占目前单光子乃至正电子核素标记的PSMA小分子抑制剂的绝大多数。

Hillier等[39]合成了4种基于尿素的PSMA小分子抑制剂:MIP-1428、MIP-1404、MIP-1405和MIP-1427,并使用99Tcm标记,随后进行了小鼠体内外的药物评价。其中99Tcm-MIP-1404表现出了在肿瘤中的高摄取以及在肾脏和非靶组织中被快速清除的能力。2014年,Vallabhajosula等[22]分别在6例转移性前列腺癌患者和健康者体内进行了Ⅰ期临床研究,进一步比较了99Tcm-MIP-1404和99Tcm-MIP-1405在人体中的药代动力学性质、生物学分布以及肿瘤摄取和转移的影像学表现。两者均显示出骨、前列腺癌和淋巴结病变的早期可视化转移灶,但在一些外分泌腺(唾液腺、泪腺和腮腺)中观察到持续摄取。99Tcm-MIP-1404显示出更少的尿排泄和更高的肿瘤放射性摄取。基于此结论,Goffin等[23]对99Tcm-MIP-1404进行了Ⅱ期临床研究,评估其在105例具有高淋巴结转移风险的前列腺癌患者中的显像表现。结果显示,前列腺病变对其的摄取与Gleason评分呈正相关(ρ=0.53,P< 0.0001),且其能在一定程度上预测淋巴结受累的情况。Reinfelder等[24]首次在60例生化复发的前列腺癌患者中应用99Tcm-MIP-1404 进行显像, 检出率为70%。 Schmidkonz等[25-27]对99Tcm-MIP-1404应用于前列腺癌的诊断和分期评估进行了一系列研究,结果显示,其在诊断肿瘤局部复发或转移、监测治疗反应以及评价前列腺癌患者的原发和分期等方面展现出较大潜力。目前,99Tcm-MIP-1404已经完成Ⅲ期临床试验,在对直肠超声活检证实的低级别前列腺癌的显像分析中,其显示出较高的灵敏度及特异度。99Tcm-MIP-1404极有可能成为首个被批准投入临床使用的靶向PSMA的放射性药物。

Kularatne等[28]合成了一系列基于2-[3-(1,3-二a羧基丙基)脲基 ] 戊二酸(2-[3-(1,3-dicarboxypropyl)ureido] pentanedioic acid,DUPA)的PSMA小分子抑制剂,标记99Tcm并在小鼠体内进行显像,其中99Tcm-DUPA表现出最高的亲和力和靶向性。该研究还揭示了间隔物的重要性,结果显示,具有7~16个原子间隔物的DUPA缀合物与PSMA有最高的亲和力,它们在肿瘤细胞中显示出强烈的摄取,除肾以外在其他组织中很少或没有累积。

另一种99Tcm标记的PSMA小分子抑制剂99Tcm-EDDA/HYNIC-iPSMA (99Tcm-ethylenediamine-N,N-diacetic acid (EDDA) / hydrazinonicotinyl (HYNIC)-Lys (Nal)-Urea-Glu)亦具有巨大的临床应用潜力。Santos-Cuevas等[29]分别对8例前列腺癌患者和健康受试者进行了99Tcm-EDDA/HYNIC-iPSMA显像。99Tcm-EDDA/HYNIC-iPSMA显示出较强的肿瘤特异性摄取,3 h平均T/NT=8.99,且该显像剂能够检测骨、软组织和淋巴结中的转移病灶。

99Tcm同样能指导手术治疗,Robu等[30]首次应用99Tcm-PSMA-I&S(99Tcm-mas3-y-nal-k-sub-KuE)进行RGS,取得了较好的效果。由于99Tcm-PSMA-I&S能够与血浆蛋白高度结合,导致其在体内清除缓慢,使得前列腺癌病变从注射后直至21 h的T/NT稳定增加。术中发现了所有在68Ga-PSMA-11 PET/CT中观察到的可疑病灶。Maurer等[40]使用99Tcm标记的PSMA小分子抑制剂进行RGS也取得了非常好的效果。相较于111In-PSMA-I&T,99Tcm价格便宜,较易获得,同时其γ能量更低,半衰期更短,使得患者和医务人员理论上的辐射暴露更少。

Maresca等[41]合成了一系列基于谷氨酸-尿素-X(X为衍生的赖氨酸)的新型卤代异二聚体PSMA抑制剂并进行体外评价,其中123I标记的MIP-1072和MIP-1095在高温下表现出极好的稳定性,在人前列腺癌LNCaP细胞中显示出对PSMA的高亲和力和选择性,这为临床开发新型药物奠定了良好的基础。随后,他们继续在小鼠体内开展123I-MIP-1072和123I-MIP-1095的显像研究。结果显示,两者均能很好的靶向结合于LNCaP细胞,在37℃时有较高的内化率,123I-MIP-1072显示出更快的血液和非靶组织清除速度;虽然123I-MIP-1095从血液和非靶组织中的清除速度较慢,但其具有更高的肿瘤摄取值。然而,两者在肾脏中的放射性浓度是所有器官中最高的,远超于它们在肿瘤中的放射性浓度。以123I-MIP-1072为例,注射2 h后,肾的最大摄取率为(159 ± 46) %ID/g,而肿瘤的最大摄取率则在注射1 h后,为(17.4 ± 6.3) %ID/g[31]。2013年,Barrett等[42]首次评估了123I-MIP-1072和123I-MIP-1095在检测前列腺、软组织和骨转移病变中的性能。7例转移性肿瘤患者的SPECT/CT结果显示,注射显像剂0.5~1 h后快速检测到了前列腺、软组织和骨中的病变。相较于123I-MIP-1095,123I-MIP-1072在注射后4~24 h区间内具有更高的T/NT,且其清除速度比123I-MIP-1095快约5倍,因此,123I-MIP-1072可用作诊断性药物。实际上,123I-MIP-1095具有更大的绝对肿瘤摄取值,但由于清除速率较慢,且其通过肾脏和胃肠道排泄的方式延长了体内清除所需的时间,因此,131I-MIP-1095可能更具有作为放射性治疗剂的潜力。

Foss等[32]合成的125I标记的N-[N-[N(S)-1,3-二羧酸丙烯酸]氨基甲酰]-S-3-吲哚-L-酪氨酸(N-[N-[(S)-1,3-dicarboxypropyl]carbamoyl]-S-3-iodo-L-tyrosine,DCIT)是第一种尿素类放射性卤化PMSA抑制剂。研究者评估了2种基于脲基的PSMA小分子抑制剂125I-DCIT和N-[N-[(S)-1,3-二羧酸丙烯酸酯]氨基甲酰]-S-[11C]甲基-L-半胱氨酸(N-[N-[(S)-1,3-dicarboxypropyl]carbamoyl]-S-[11C]methyl-L-cystein, 11C-DCMC)和在小鼠体内的显像效果,结果显示,注射显像剂30 min后,两者均在LNCaP细胞异种移植小鼠中显示出明确的靶向图像且在肾脏中观察到较高浓聚,但11C-DCMC显示出更高的T/NT(10.8 vs. 4.7)。

在之前研究的基础上,Zechmann等[33]通过124I-MIP-1095 PET/CT显像评估单周期的131I-MIP-1095(平均活性:4.8 GBq,范围:2~7.2 GBq)在男性mCRPC患者中的潜在治疗用途以及安全性和有效性。患者在核医学治疗病房中接受5~7 d的治疗,治疗前和治疗中均以高氯酸钠、柠檬汁和冰袋等多种方式减少器官灌注。结果显示,60.7%的患者PSA水平降低>50%;84.65%的骨痛患者疼痛完全或中度减轻;25%的患者出现短暂的轻微至中度口干;淋巴结和骨转移的吸收剂量高达300 Gy。显像剂在唾液腺、肝脏和肾脏中的摄取值较高(平均摄取剂量分别为4.6、1.5和1.5 mSv/MBq),甲状腺的平均摄取剂量为0.9 mSv/MBq。随访期内未观察到对肾功能的不良影响,治疗后肝功能和甲状腺激素的分泌没有变化。1例患者在使用甲状腺阻滞剂后出现恶心、呕吐并甲状腺功能减退。虽然口干的症状是暂时的,但放射性药物在腺体内的高剂量摄取仍然值得关注,研究者试图通过使用冰袋和柠檬汁来减少腺体对放射性药物的吸收,但目前尚不清楚这些预防措施对患者的腺体功能有何影响。骨髓抑制是研究中观察到的最为主要的不良反应,对于这个结果,研究者认为这些患者在进行131I治疗之前已经接受了放化疗,所以131I所造成的具体不良反应及其大小不易评估。相较于90Y和177Lu标记的抗体J591,131I的治疗优势更大[43]。

Hillier等[44]对131I-MIP-1095的类似物131I-MIP-1466在小鼠前列腺癌移植瘤中的治疗效果进行了研究,结果表明,适当剂量的131I-MIP-1466能够抑制异种移植瘤的生长。此外,131I-MIP-1072的类似物131I-MIP-1375在人前列腺癌肿瘤模型中治疗功效的研究也得到了相同的结论[45],这为131I-MIP-1466和131I-MIP-1375应用于人前列腺癌的靶向放射治疗提供了基础。以上两种放射性治疗剂目前仅应用于动物实验,尚未进行临床研究。
在我国,131I应用于医学治疗的技术和方案已经成熟,很多地方也已经建立了标准化病房,对于其效果和不良反应也有深刻的认识。目前,前列腺癌的放射性核素治疗多使用177Lu标记的PSMA小分子抑制剂,其中177Lu-PSMA-617显示出了良好的治疗效果和安全性[46-50]。然而,177Lu较难获得且国内临床经验不足,相较之下,应用131I进行治疗似乎更符合中国国情。同时,有研究结果表明177Lu标记的PSMA小分子抑制剂对一些患者治疗无效[51-53],这提示研发其他β放射性核素标记的PSMA小分子抑制剂是有必要的。

Maurer等[34]首次使用111In-PSMA-I&T进行了前列腺癌的RGS治疗,术前24 h向患者体内注射146 MBq的111In-PSMA-I&T,术中用γ探针检测肿瘤,结果显示,术前PET/CT检出的肿瘤在术中均被找到,且术后组织学分析结果证实切除组织中的PSMA表达均呈阳性,这证实以PSMA为靶点的RGS可以提高手术效果。Rauscher等[54]回顾性分析了31例局部复发性前列腺癌患者接受使用111In-PSMA小分子抑制剂进行的RGS,结果证明,111In-PSMA RGS对于行挽救性淋巴结切除术的前列腺癌患者以及小转移性病灶的术中检测具有重要价值。Mix等[18]的研究结果表明,111In-PSMA-617具有较高的区分受影响和未受影响的淋巴结的能力,其准确率为97.7%(215/220)。

摘要: 前列腺特异膜抗原(PSMA)在前列腺癌中特异性过表达,以其作为生物靶点,为前列腺癌的临床诊断和治疗提供了新的策略。目前,对PSMA小分子抑制剂标记的研究多集中于应用正电子核素如68Ga、18F等进行标记并通过PET显像实现对前列腺癌的诊断,或应用β放射性核素如177Lu进行标记以靶向治疗前列腺癌。近年来,应用单光子核素标记PSMA小分子抑制剂的研究也取得了进展。笔者就单光子核素标记的PSMA小分子抑制剂在前列腺癌中的临床应用进展进行综述。

Abstract: Prostate-specific membrane antigen (PSMA), highly expressed in prostate cancer, provides a new strategy for the clinical diagnosis and treatment of prostate cancer as a specific biological target. At present, the research on the labeling of PSMA small molecule inhibitors is mostly focused on positron nuclide labels, such as 68Ga and 18F or beta radionuclide labels, such as 177Lu to realize diagnosis and targeted therapy of prostate cancer. In recent years, the application of single photon nuclide-labeled PSMA small molecule inhibitors has also made progress. This review summarizes the clinical application of single photon nuclide-labeled PSMA small molecule inhibitors in prostate cancer. -
