基于FAP靶点的放射性碘难治性甲状腺癌诊疗一体化进展

Advances in theranostics based on the FAP target for radioactive iodine-refractory thyroid cancer

  • 摘要: 成纤维细胞活化蛋白(FAP)在多种上皮肿瘤相关成纤维细胞中高度表达,但其在正常组织中的表达较低。因此,FAP被认为是具有潜力的肿瘤显像和治疗的靶点。部分分化型甲状腺癌患者会进展为转移性碘难治性甲状腺癌(mRAIR-TC),mRAIR-TC一般使用酪氨酸激酶抑制剂(TKI)进行治疗,但部分患者经TKI治疗后仍然会出现疾病进展,这类患者的临床管理仍是比较大的挑战。为明确FAP靶向策略在放射性碘难治性甲状腺癌诊疗一体化中的潜在价值,笔者基于临床转化研究的最新进展,对其在该类患者中的应用价值进行综述。

     

    Abstract: Fibroblast activation protein (FAP) is highly expressed in cancer-associated fibroblasts in various epithelial tumors, but its expression is low in normal tissues. Therefore, FAP is considered as a potential target for tumor imaging and therapy. Some patients with differentiated thyroid cancer progress to metastatic radioactive iodine-refractory thyroid cancer (mRAIR-TC). mRAIR-TC is generally treated with tyrosine kinase inhibitor (TKI), but disease progression still occurs in some patients after TKI treatment, and the clinical management of these patients remains a major challenge. To clarify the potential value of FAP-targeted strategies in the theranostics of radioactive iodine-refractory thyroid cancer, the authors review their application value in such patients, based on the latest advances in clinical translational research.
